LIABILITY WAIVERS
All participants of Teen Street Skills will be required to agree to release forms (liability and media). Parents enrolling their kids must agree to the parent form and teen/minor form, while participants ages 19 and older signing up for themselves may agree to the participant form. Please click the links in the previous sentence to review the full agreements. You may also review the full photo/video form.
NO SHOWS & LATE CANCELLATIONS
Cancellations must be made 48 hours prior to scheduled class for a refund.
Teen Street Skills is a non-profit corporation, so the refundable registration fee has been put in place strictly to protect against NO SHOWS. NO SHOWS waste valuable charity resources, including the cost of vehicles, programs assets, professional instructors, and staff…there are only so many openings for each program, each spot is very valuable. NO SHOWS and/or late cancellations turn this great opportunity into an empty seat that could be filled by a willing teen from the waitlist.

I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video tape without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording. I also understand that this material may be used in settings within an unrestricted geographic area.
Photographic, audio or video recordings may be used for the following purposes:
I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in the public for promotional purposes.
There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.
This release applies to photographic, audio or video recordings collected as part of the sessions listed on this document only.
I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for educational and promotional purposes.
* If this release is obtained from a participant under the age of 19, then the participant’s parent or legal guardian is required.
